At CommuniCare+OLE, we offer a variety of services to treat multiple substance use disorders (SUDs), including OUD (opioid use disorder), AUD (alcohol use disorder), and StUD (stimulant use disorder).
Medication-Assisted Treatment (MAT)
MAT uses FDA‑approved medications to treat opioid and alcohol addiction. Coupled with counseling, MAT treats the whole patient and significantly reduces the rate of relapse. Medi‑Cal, Medicare, and many private insurance plans cover MAT.
Our MAT team is multidisciplinary and includes administrative support, peer support, behavioral health treatment, prescription medication support, and other innovative treatments for SUD. We partner directly with primary care providers, local emergency departments, and other community resources for continuity of care. Clients enrolled in MAT are offered customized treatment plans including individual counseling and outpatient programming. Youth 15 years and older can access MAT services with parental consent.

- Prescription medications such as Buprenorphine (commonly known as Suboxone, Sublocade, Subutex, etc.), Naltrexone, or Vivitrol) and other medications are part of treatment options for substance use disorders
- Mental health counseling
- Outpatient groups and individual supports
- Access to harm reduction services, including Narcan and fentanyl testing strips
Outpatient Substance Use Treatment
CommuniCare+OLE offers Outpatient Substance Use Disorder Services in Woodland and West Sacramento for adults and adolescents. We also offer specialized outpatient services for dually diagnosed adults, pregnant and/or parenting mothers, and for individuals who need medication assistance to stop opiate and alcohol use.
Substance Use Services
- Access to Medications for Addiction Treatment (including Naltrexone, Vivitrol, and Suboxone)
- Screening appointments to enroll in county-funded services
- Weekly in-person substance use disorder groups, counseling, and care coordination
- Substance use education in English and Spanish
- Guidance and support from a Peer Advocate or SUD Navigator with lived experience
- Evidence-based treatments for stimulant use (incentives)
Walk-in Appointments
Walk-in appointments at our West Sacramento and Woodland offices are available Monday – Friday, 9:00 a.m.–4:00 p.m. You can also call the 24/7 Yolo County Access line at 888.965.6647.
- ASAM access screening: Available at Hansen, Salud, or Nav Center during business hours by appointment or walk-in
- Outpatient Substance Use Recovery Program: Available in English or Spanish in West Sacramento and Woodland
- Co-occurring disorder treatment: Available in West Sacramento and Woodland*
- In-custody substance use disorder provided to clients in Yolo County custody
- Intensive outpatient treatment: Services available in Woodland*
- Perinatal intensive outpatient treatment: Substance use services for pregnant or parenting women — free transportation and childcare is provided*
- Recovery Incentive Program for people who have a stimulant use disorder
- Medication-Assisted Treatment (MAT): Referrals can be made directly and do not need an ASAM access screening
* These services must start with ASAM access screening.
Youth Opioid Response (YOR) is a free resource for anyone who is struggling with opioid, stimulant, or alcohol use disorder and needs extra support. YOR provides drug education, prevention, and intervention services, as well as a connection to housing resources, employment, legal help, and more.
