SALARY RANGE:

  • ASW/AMFT/APCC: $71,760.00 to $75,337.60 annually
  • LCSW/LMFT/LPCC: $81,473.60 to $99,590.40 annually

JOB SUMMARY: The Behavioral Health Clinician for Youth Services provides individual, group and family therapy to clients in various treatment programs. The CommuniCare Health Center’s mission emphasizes supportive, nonjudgmental care, preventive care, health education and community referrals to address the broad spectrum of health and psycho-social concerns of clients served. CommuniCare Health Centers is a Federally Qualified Health Center providing comprehensive patient-centered primary care services including medical, dental, substance abuse treatment, behavioral health and outreach services to low income, multi-ethnic, uninsured and underinsured populations.

EDUCATION, EXPERIENCE, TRAINING

  1. ASW/LCSW, AMFT/LMFT, APCC/LPCC required. Must be in good standing with the Board of Behavioral Science
  2. 2 years minimum working in the behavioral health/mental health field
  3. Bilingual preferred; English/Spanish

KNOWLEDGE OF WORK

1. Effectively utilize measurement-based care practices to determine client progress, wellness and risk factors; collect required departmental pre/mid/post health screenings (e.g., PHQA, CANS).
2. Ability to address a wide range of mental health conditions, utilize a variety of assessment tools, facilitate diagnostic evaluations and treatment interventions to address client needs, goals, and circumstances.
3. Competently deliver therapy services to youth and families with client goals being at the center of care.

DUTIES AND RESPONSIBILITIES

1. Conducts comprehensive intake assessments and client planning sessions for youth clients and their families.
2. Provides individual, group and family therapy in clinic locations and clients’ homes, as well as in the community throughout Yolo County.
3. Completes all client information and documentation in a timely manner according to legal, ethical, funding and agency guidelines
4. Works with clients to establish long- and short-term goals.  Counselor is responsible for detecting emotional, psychological, and physical danger signals.  Counselor is non-judgmental and offers feedback and support to clients
5. Provides case management, including linkage and referral, crisis intervention, rehabilitation, safety planning and collateral services for youth.
6. Provides information to clients including treatment alternatives and referral service.
7. Responsible for remaining within the ethical standards established in the field, including those of confidentiality.
8. Responsible for explaining and enforcing all rules and regulations associated with clinic treatment policy.
9. Compile and report data as required.
10. Collaborate with Probation, Juvenile Detention Facility staff, CWS and other agencies to case manage clients as directed by supervisor.
11. Attend clinical supervision and weekly staff meetings, as necessary.
12. Attend training/conferences as arranged.
13. Responsible for maintaining the standards as listed in the CommuniCare Health Centers Personnel Policies and Procedures Manual, and any other standards which may apply to this position.
14. Other projects as may be assigned.
