At CommuniCare+OLE, we put the patient at the center of everything we do. And we view primary care as more than just medical care; it includes an array of services like dental, behavioral health, healthy living programs, care coordination, and more. 

When you are a patient here, you have a dedicated care team, led by a primary care provider (PCP), who works across our various departments and with you to help you lead the healthiest life possible.

This model of care is one of the reasons we are designed as a Patient-Centered Medical Home by the National Committee for Quality Assurance (NCQA) and a quality leader by the Health Resources and Services Administration (HRSA).

Part of a PCMH designation is a commitment to continuous improvement. Our Quality Improvement team works to ensure that our patients receive necessary screenings, immunizations, and preventative interventions.

A Commitment to More than Medicine

Here are just some of the offerings at CommuniCare+OLE that set us apart from other health systems:

  • Warm handoffs Our health center staff works as one big team to try to address as many concerns as possible during your visit. If you come in for medical care and express other concerns, we aim to connect you that day to our other services with a warm handoff, often in the exam room, that same day. Whether you need behavioral health support for life’s stressors, dental care, food and nutrition counseling, or clinical pharmacy, we do our best to make a connection then and there. 
  • Care coordination Care coordinators help our patients access resources both within and outside of CommuniCare+OLE to help them address issues like food security, transportation access, safe housing, and more. Your care coordinator is part guide, part coach, and part cheerleader on your health journey. 
  • Group visits We offer group visits for patients with diabetes and pediatric patients. These classes include a one-on-one check-in with the patient’s provider and educational information on healthy eating, physical activity, medication, and more. 
  • Enrollment services Individuals who need assistance enrolling in state plans like Medi-Cal or CalFresh can get one-on-one application assistance from our team of trained enrollment professionals.
