Current Groups — Behavioral Health

If you are interested in joining one of the groups below, please speak with any CommuniCare+OLE provider to get more information or a referral, if needed.

Depression and Anxiety Group (English)

This group is for patients and clients who have received care from a behavioral health clinician in one of our health centers and is focused on helping manage depression and anxiety. This group is offered:

  • Salud Clinic (West Sacramento) every Friday, 10:00–11:30 a.m. 

Current Groups — Substance Use Treatment

We offer outpatient substance use disorder treatment for adults in Yolo County. Treatment services offer evidence-based practices such as cognitive behavioral therapy, motivational interviewing and other interventions including Peer Support, Incentives and Medications for Addiction Treatment.

All SUD Treatment groups are open to Yolo County residents on Medi-Cal (Partnership) or residents who are referred by a Yolo County organizations such as Probation or Child Welfare. There are also options for self-pay based on a sliding scale for uninsured or out of county participants.

Participants must complete a level of care placement screening prior to enrollment by calling 888.965.6647 or walking into the clinics Monday – Friday between 9:00 a.m.–4:00 p.m.

Intensive Outpatient Groups

Adult Intensive Outpatient Group for Substance Use Disorder (SUD) (English)

This group is for adults in substance use recover and offers a variety of skill building tools and resources, peer support, relapse prevention, cognitive behavioral therapy (CBT), and emotional regulation. This group is offered:

  • Hansen Family Health Center (Woodland) every Monday, Wednesday, Thursday, and Friday, 9:00 a.m.–12:00 p.m. 

Journey HOME (English)

Journey HOME (Healing through Outpatient treatment and Maternal Education) offers intensive outpatient substance use treatment for maternal caregivers pursuing recovery from addiction to alcohol and other drugs.  This group meets 4 mornings a week for 3 hours each day and is designed to support the recovery process.  The program provides transportation and childcare to eliminate barriers for engagement. The group is open-ended and welcomes up to 12 participants. This group is offered:

  • Salud Clinc (West Sacramento) every Monday, Tuesday, Thursday, and Friday, 9:00 a.m.–12:00 p.m.

Outpatient Groups

Outpatient treatment includes a combination of group and individual counseling for participants who want to receive support for their substance use but need a more flexible schedule. 

Path to Recovery (P2R) (English)

These groups are focused on mental health and substance use. Facilitators offer tools for seeking safety, dialectical behavioral therapy (DBT), relapse prevention skills building, and peer support.

  • Hansen (Woodland) every Monday, Wednesday, and Friday, 1:00–2:30 p.m.
  • Salud (West Sacramento) every Monday, Wednesday, Thursday, 1:00–2:30 p.m.

Outpatient Substance Use Recovery Program (English)

Groups are for individuals in substance use recovery and focuses on relapse prevention, skill building, healthy boundaries, MRT, cognitive behavioral therapy, and peer support.

  • Salud (West Sacramento) every Monday, Wednesday, and Friday, 4:00–5:30 p.m.
  • Hansen (Woodland), Monday, Tuesday, Wednesday, Thursday, and Friday, 4:00–5:30 p.m.

Nuevos Comienzos (Spanish)

Outpatient substance use treatment for Spanish speakers. Groups are conducted twice per week using culturally relevant approaches and materials in Spanish focused on education about drugs and addiction, relapse prevention, skill building, and resources. Individual counseling included.

  • Hansen Family Health Center (Woodland) every Monday and Wednesday, 4:00–5:30 p.m.

Current Classes

Group Visits for Diabetes

Group visits for patients with diabetes are available at several of our health centers in Yolo County and at our North Napa Health Center on Pear Tree Lane. Group visits provide you with an opportunity to see your provider and participate in educational sessions to help you manage your diabetes and overall health. Group visits are offered several times a month:

  • Hansen on 1st and 3rd Thursdays, 1:00–2:30 p.m. (Spanish language)
  • Hansen on 2nd and 4th Wednesdays, 5:00–6:30 p.m. (English language)
  • DCC on 1st and 3rd Fridays, 1:00–2:30 p.m. (Spanish language)
  • DCC on 2nd and 4th Wednesdays, 1:00–2:30 p.m. (English language)
  • Salud Clinic on 1st and 3rd Fridays, 1:00–2:30 p.m. (Spanish language)
  • Salud Clinic on 2nd and 4th Fridays, 1:00–2:30 p.m. (English language) 
  • North Napa Campus on 1st, 2nd, and 3rd Mondays 4:00–6:00 p.m. (Spanish language)

Prenatal Classes

Several prenatal and postpartum classes are offered at our Davis Community Clinic, including:

  • Childbirth 
  • Infant care 
  • Postpartum 
  • Breastfeeding 
  • Baby wearing
  • Car seats
