Find Resources Inside & Outside of CommuniCare+OLE
CommuniCare+OLE has many great services and resources for our patients. However, knowing what is available and how to access it can be a challenge. That’s why we have bilingual care coordinators to help you find resources both inside and outside of CommuniCare+OLE.
Our care coordinators work with your team of providers to help you get the most from your care at CommuniCare+OLE. They can assist with:
- Scheduling appointments
- Filling out forms
- Securing transportation to and from appointments (restrictions apply)
- Providing support to help you manage your health and stay on track with provider recommendations between appointments
- Navigating community resources
If you need services that CommuniCare+OLE doesn’t provide, we will put you in touch with one of our referral coordinators to help connect you.
Enhanced Care Management (ECM)
Enhanced Care Management (ECM) is different from Care Coordination. It is a statewide, specifically for Medi-Cal managed care members who meet certain criteria. Eligible members are assigned a Lead Care Manager who will meet them wherever they are: on the street, in a shelter, in our health centers, or at home.
Through Enhanced Care Management, members can also be connected to Community Supports services to help address their health-related social needs, such as access to healthy foods or safe housing to help with recovery from an illness.
Eligible individuals include:
- Adults, unaccompanied youth and children, and families experiencing homelessness.
- Adults, youth, and children who are at risk for avoidable hospital or emergency department care.
- Adults, youth, and children with serious mental health and/or substance use disorder needs.
- Adults living in the community and at risk for long-term care institutionalization.
- Adult nursing facility residents transitioning to the community.
- Children and youth enrolled in California Children’s Services (CCS) or CCS Whole Child Model with additional needs beyond their CCS condition(s).
- Children and youth involved in child welfare (foster care).
- Adults and youth who are transitioning from incarceration.
- Pregnant and postpartum individuals.
Resources at CommuniCare+OLE
Communicare+OLE offers several food distribution programs and services across our communities.
Fruits & Veggies Distribution
If you have a hard time getting access to fruits, veggies, and other whole foods, CommuniCare+OLE has resources to help you. We offer FREE food distributions the third Friday of every month:
- North Napa Campus (1141 Pear Tree Lane, Napa): 2:00–4:30 p.m.
- Calistoga (911 Washington Street, Calistoga): 2:30–4:00 p.m.
- Fairfield Adult School (900 Travis Blvd, Fairfield): 2:00–4:00 p.m.
Distributions include seasonal produce, dry goods, and pet food courtesy of Jameson Humane. Distributions run until the noted end time or until food runs out.
Produce Rx in Yolo
Produce Rx enables patients with high blood sugar to access fruits and vegetables as part of their healthcare services at CommuniCare+OLE. Participants receive produce valued at $70 per month. This program is made possible through generous support from the USDA and the commitment of our local partners: Nugget Markets, Spork Food Hub, the Davis Farmers Market, and the Center for Land-Based Learning‘s Mobile Farmers Market. Ask your provider for information on how to enroll.

CalFresh Enrollment
CalFresh is a state government program that provides money to pay for nutritious food for those who qualify. This extra money can be used to buy food at grocery stores, farmers’ markets, and even online for groceries from certain retailers.
CalFresh dollars can be used on almost any food item with very few restrictions. The amount of support available varies based on household size and income, and CommuniCare+OLE can help you learn if you’re eligible and apply if you are.
