How do I access the patient portal?
To begin using the CommuniCare+OLE patient portal in Napa and Solano Counties, you will need to request access. You can do that during an appointment or by calling 707.254.1770. You will receive an automated email with login instructions. Make sure to check your spam folder if you don’t see it.
For Yolo County patients, please contact your clinic or ask any CommuniCare+OLE team member at your next appointment.
How do I request my medical records?
You can download your medical records, progress notes, and other health information via the online patient portal.
You can also request copies of health information from the Health Information Department by completing the Release/Request of Information form:

How do I get my vaccine records?
We report all vaccines to the State Department of Health. You can request your digital vaccine record online from the state of California at anytime by visiting https://myvaccinerecord.cdph.ca.gov/ and filling out the requested information.
How can I schedule a medical appointment?
If you are a new patient, you can call us at 707. 254.1770. If you are an existing patient, you can schedule an appointment by calling or by sending a message online through our patient portal.
In Yolo County, please call one of our facilities below. You can also use our patient portal to make your appointments online and access your health information as needed. We will send you reminder texts or calls seven, three, and one day(s) before your scheduled appointment time.
How do I get a refill for my medication?
To request a prescription refill, please call the pharmacy where you picked up your medicine. The phone number is on your prescription bottle. If no refill is available, please call 707.254.1770. Once we receive your request, your prescription will be ready within 3 business days.
How will I know my lab results are ready?
Your care team will contact you when your lab results are in. If you’re registered with the portal, you can review your lab results as soon as they’re ready.

Does CommuniCare+OLE offer assistance to the uninsured?
Yes. If you are uninsured, we charge for services on a sliding fee scale, based on your family income. We can also help you apply for insurance or reapply if you have lost coverage. Please call us at 707.254.1777.
Can I pay out of pocket?
You are not required to have health insurance to receive services at CommuniCare+OLE. Sliding fee discounts are available to all patients who meet family size and income requirements.
Does CommuniCare+OLE accept Medi-Cal?
CommuniCare+OLE accepts most health insurance plans and coverage including (but not limited to) Medi-Cal, Partnership Health Plan, Medicare, County Medical Services Program (CMSP), Nivano, Denti-Cal, and River City Medical Group. Our trained enrollment specialists can help you with the Medi-Cal application process. If you are already enrolled in a Medi-Cal plan, we’ll assist you with the necessary forms or steps to enroll in our care programs.
I am an undocumented person. Are CommuniCare+OLE services available to me?
CommuniCare+OLE sees all patients regardless of immigration status.
Behavioral Health
Do you accept Medi-Cal?
Yes, we accept Medi-Cal for primary and behavioral health services.
Can I pay out of pocket?
We have a sliding scale fee for services based on income. We will not turn anyone away for inability to pay.
Does CommuniCare+OLE have services in different languages/ translation services?
Yes, CommuniCare+OLE Health Centers have a translation service on hand. Please mention which language you would like when you make your appointment to help us prepare in advance!
Do you offer LGBTQ-inclusive services and resources?

Do you provide couples counseling?
Yes, if you have an open case with CWS or Cal Works in Yolo County.
Do you provide mental health counseling?
Yes, if you receive your primary care with CommuniCare+OLE Health Centers you can get counseling for free through Integrated Behavioral Health (IBH).
Pediatrics
Do you have pediatricians?
Yes. We also have family medicine providers who see children as well as adults. You can learn more about our providers here.
What is pediatric care?
Pediatric care includes health and illnesses of infants, teens, and young adults. CommuniCare+OLE’s clinicians provide preventive healthcare for children in good health and medical care for children who are acutely or chronically ill. We also provide parents with support and advice with issues such as growth and development, safety and prevention, nutrition, and emotional wellness to foster a lifetime of good health.
What are well-child visits and how often should my child be seen?
Your child should not only seek care for an illness. It is also important to schedule well-child-care exams regularly, beginning in infancy. Also called well-care visits or checkups, these routine examinations provide the best opportunity for the doctor to observe the progress of your child’s physical and mental growth and development; to counsel and teach parents; to detect problems through screening tests; to provide immunizations, and to get to know one another. Well-care visits are strongly recommended as part of preventive pediatric care.
Well-child visits are also a good time for parents to raise questions and concerns about a child’s development, behavior, nutrition, safety and overall well-being.
CommuniCare+OLE recommends this schedule for routine well-care visits:

Why does my child need to receive vaccinations?
Immunizations are a series of shots given to children at different ages to help ward off serious, and potentially fatal, childhood diseases. Making sure your child receives immunizations when scheduled is the best way to help protect your child from potentially fatal diseases. According to the American Academy of Pediatrics, vaccinations have reduced the number of infections from vaccine-preventable diseases by more than 90%. If you’re apprehensive about vaccinations, please do not hesitate to contact our office.
What should I bring to my child’s first visit?
Please provide the following information when you arrive for your visit:
- Insurance cards
- Complete immunization records
- Medical records from your child’s previous medical provider (if applicable)
What should I do if I need to see a medical provider after hours?
For most office visits we encourage you to make an appointment. However, we always strive to see our patients the same day for sick visits. We have an on-call service and there is always a medical provider available to assist you. Simply call our office number and you will be connected to the nurse triage service that will be happy to assist you.
Remember, if you have a life threatening emergency, call 911 immediately.
Perinatal Care
Do I need insurance for perinatal services?
There is no need to apply for or have insurance before your first appointment. Our trained enrollment specialists will help you with the Medi-Cal application process. If you are already enrolled in a Medi-Cal plan, we can help you complete any forms or additional steps necessary to enroll in CommuniCare+OLE’s Perinatal Program. No one is ever turned away for care based on ability to pay. We can begin your care at any stage of pregnancy.
Do I continue seeing my primary care providers while I’m receiving perinatal care?
Often you will only need to see your perinatal care team during your pregnancy; however, there may be some situations where you would continue to see your primary care providers for conditions unrelated to pregnancy. Also your midwives will refer you as needed to any specialists, dental care, and behavioral health services. Whatever you may need in your pregnancy or postpartum, we can either provide it directly for you or help arrange it for you.
What can I expect while giving birth at the Sutter Davis Birthing Center?
- 24/7 nurse midwifery care
- Private labor and postpartum rooms featuring calm, adjustable lighting, a quiet environment, private shower for you and your labor support team. Some rooms include a birthing tub for water births.
- Partner and family members are welcome to be present during labor and birth
- Volunteer doula services are available
- Breastfeeding support and education
Can I tour the Birthing Center before it’s my time to deliver?

I am in a nontraditional parenting relationship and experiencing my first pregnancy. How will we fit in with perinatal care services?
The midwives and perinatal team at CommuniCare+OLE and Sutter Davis Hospital respect all forms that families take, including LGBTQIA+, and invite you to include whomever you consider important to your care to join in your prenatal visits and to be a part of your birth team (adherent to current visitor policies).
Can I meet my child's provider before my baby is born?
Yes, in fact we strongly encourage parents-to-be to meet our pediatric providers during pregnancy. During this visit, we will answer any questions that you have about our practice or your new child. Ask your prenatal provider about scheduling such a visit if you are interested. Some of our pediatric providers are also able to care for all members of the family (including parents and adults). If you would like to have the same postpartum primary care provider as your baby, please be sure to let us know.
Sexual Health
Is my visit to a clinic confidential?
Yes, we offer free and confidential (private) sexual and reproductive health services to patients ages 12 and older.
What information will I be asked to share?
To provide you with the best possible healthcare, our providers will ask detailed questions about your sexual health and activity. Our providers are trained to have these conversations in a nonjudgmental, noncoercive way. You always have the right to refuse to answer questions or give information to your provider if you are not comfortable.
I am under 18. Do I need my parent’s permission to seek services?
- For sensitive services, such as birth control or sexual health testing and treatment, you can see a medical provider without your parents’ permission if you are at least 12 years old.
- You can make your own appointments by calling our clinics or walk into any CommuniCare+OLE clinic in Yolo County and fill out a Safe Care Clinic confidential visit form at the front desk.
- If you are stressed, be open and honest at your appointment. We are here to help you.
- These visits are strictly confidential, just between you and the medical provider.
- You can have a private appointment by phone if you aren’t able to come to a clinic.
What if I think I'm pregnant?
We offer pregnancy testing and options counseling to provide the best plan for you. Call one of our clinics as soon as you think you are pregnant.
Do you offer abortion services?
No — our staff can help connect you with an abortion provider if that is your choice.
What is an emergency contraceptive pill (ECP, also known as "Plan B" or "morning after pill”)?
Emergency contraception is a way to prevent pregnancy after unprotected sex. It works by delaying the release of an egg during the monthly cycle. However, if the sperm and egg unite and travel together to attach to the uterine lining, ECP will not be able to interrupt the pregnancy.
Every Safe Care Clinic offers ECP at no cost.
How soon can you test for sexually transmitted infections (STIs) after sexual activity?
After engaging in sexual activity of any kind (oral, vaginal, anal), you should test for STIs in two weeks.
How often should I get tested for STIs?
You should get tested for STIs at least once per year while sexually active, and more often if you fall into certain groups. See the CDC website for more details: https://www.cdc.gov/std/prevention/screeningreccs.htm
How often should a woman get a pelvic exam and Pap test?
Pap tests are recommended every three years, starting at age 21 until age 65. More frequent Pap tests may be needed if an abnormal test result is found or if you are at high risk of cervical cancer.
What other sexual and reproductive health services are available from CommuniCare+OLE?
- Annual gynecological exams and Pap tests
- Mammogram referrals
- Menopausal care, including hormone therapy
- Treatment for urinary tract and vaginal infections
What are PrEP and PEP medications, and who should take them?
The PrEP (pre-exposure prophylaxis) pill is taken daily and can reduce the risk of HIV contraction from sex (by 90%+) and shared needles (by 70%+). PrEP is most effective when taken for at least 7 days. It’s prescribed for those who are HIV negative and share needles, and/or have condomless sex with a partner of unknown or positive HIV status.
The PEP (post-exposure prophylaxis) pill is taken within 72 hours of possible HIV exposure, and daily thereafter for 28 days. PEP can reduce the risk of HIV transmission by almost 80%. Prescribed for those who are HIV negative and have had condomless sex with a partner of unknown HIV status, shared needles, experienced sexual assault, or had a workplace injury.
Where can I obtain a PrEP or PEP prescription?
Both PrEP and PEP are available at our pharmacy and from CommuniCare+OLE’s Safe Care Clinic in Yolo and through any of our other healthcare providers. A 60-day supply of PrEP and a 28-day supply of PEP are available without a prescription, and both pills are covered under Medi-Cal. We provide free and confidential HIV testing and STI testing.
Vaccines
How can I get a vaccine?
CommuniCare+OLE is now vaccinating eligible patients and community members ages 6 months and older. Schedule your appointment here.
Who can get a vaccine at CommuniCare+OLE?
All community members ages 6 months and older are eligible. You do not have to be a current CommuniCare+OLE patient.
Should I get the vaccine?
Yes, unless your medical provider directs you otherwise. The vaccine will help protect you, your family, and our community from getting sick with COVID-19.
Are the vaccines safe?
Yes. The vaccines were tested on tens of thousands of people in clinical trials. They have been approved by the U.S. Food and Drug Administration (FDA).
How much does it cost to get a COVID-19 vaccine?
COVID-19 vaccines are free.
Will I get sick from the vaccine? What are the side effects?
You might feel sore in your arm where you get the shot. You may feel fever, headache, or muscle aches for a day or two. Short-term side effects are usually a sign the vaccine is working. Serious side effects are very rare. If you have concerns, please talk to your medical provider.
Do I need the COVID-19 vaccine if I have already had COVID-19?
Yes. People who have been previously infected can be infected again.
Which vaccine will I get?
Currently, CommuniCare+OLE is administering the Pfizer, Johnson & Johnson, or Moderna vaccines. After you have your vaccine, you will be given a card that shows the type of vaccine you received along with the date(s).
Do I have to get the second dose?
Both doses are needed for full protection from the virus for Moderna and Pfizer. You should get your second dose at the same place you received your first dose. Second doses of Moderna vaccine are typically administered 28 days after the first dose. Second doses of Pfizer vaccine are typically administered 21 days after the first dose.
Can I still get COVID-19 after being vaccinated?
Yes. Some people have been getting “breakthrough” COVID-19. However, vaccinated people are more likely to have mild cases or not have any symptoms at all. Vaccination and boosters make it much less likely you will be hospitalized or die from COVID-19.
Do I need to keep wearing a mask if I am vaccinated?
Yes. It is recommended that you continue to:
- Wear a mask. A surgical, KN95, or N95 mask is recommended as cloth masks are less effective at preventing the spread of more contagious variants.
- Practice social distancing
- Avoid gatherings and crowds
- Wash your hands often
Do I need to get a booster shot?
The CDC recommends boosters everyone eligible get a booster specifically formulated for the omicron variant, also know as a bivalent booster. When to get a booster and which booster to get depend on age.
- Children 5-11 are eligible for a Pfizer monovalent booster if it has been 5 months or more since completing the two-dose primary series.
- Individuals 12 and older are eligible to receive the Pfizer bivalent booster 2 months after completing a primary vaccine series or receiving a monovalent booster.
- Individuals 18 and older are eligible to receive the Moderna bivalent booster 2 months after completing a primary vaccine series or receiving a monovalent booster.

How do I know which vaccine I got?
On your vaccine card, there is a column for Product Name/Manufacturer. The name of the vaccine manufacturer will be written in that box.
How long after my second shot can I get a booster?
Boosters are available for children 5-11 years old five months after your second two-dose vaccine. Boosters are available for those 12 and older 2 months after completing a primary vaccine series or receiving a monovalent booster.

Is the booster free?
Yes, COVID-19 vaccination, including boosters, is free.

What is the difference between a booster and a third shot?
A booster is an extra smaller dose of the vaccine given to maintain protection from infection over time. A third shot is an extra full dose of the vaccine that is given to people whose immune system may be weaker than normal because of cancer or other issues.
What is a bivalent booster?
The original COVID-19 vaccine and booster were formulated to work on the original variant of the virus. The new, bivalent boosters are formulated to work on the original variant of the virus and the omicron variant, which has become the dominant strain. The hope is that this increases protection against the dominant strain of the virus.
Do I need a bivalent booster if I am already boosted?
It is recommended that anyone eligible receive the new, bivalent booster shot.
Am I going to need a second bivalent booster?
At this time, there is no recommendation to get a second, bivalent booster. However, the recommendations could change depending on future variants or changes with the virus transmission.
