By registering for this event, I authorize OLE Health to use my picture, including photographic and video images, and my voice. I hereby grant to OLE Health the right to use, publish, and reproduce, for all purposes, my name, pictures of me in film or electronic (video) form, sound and video recordings of my voice, in printed and electronic copy in any and all media including television and the Internet, and for exhibition, promotion, advertising, press conferences, meetings, conferences and in brochures and other print media.
