Behavioral Health Programs Specific to Yolo County
CommuniCare+OLE has grants with Yolo County to provide certain, additional behavioral health services to Yolo County residents. This is an overview of several of those programs.
Adult Substance Use Disorder Treatment
For adults struggling with Substance Use Disorder, we offer screening and treatment, including:
- ASAM Screening and Access: Available at Hansen or Salud, during business hours by appointment or walk in.
- Outpatient substance use recovery program: Available in English or Spanish in West Sacramento and Woodland.
- Co-occurring disorder treatment: Available in West Sacramento and Woodland.*
- In-custody substance use disorder provided to clients in Yolo County custody.
- Intensive outpatient treatment: Services available in Woodland*
- Perinatal intensive outpatient treatment: Substance use services for pregnant or parenting women. Free transportation and childcare is provided.*
- Medication assisted treatment (MAT): Referrals can be made directly without ASAM Access Screening
*Service must start with ASAM Access screening
Community Mental Health Services
- Specialty mental health assessments
- Peer support and re-entry services
- Mental Health Diversion programs: Referrals made by District Attorney’s Office and Homeless Outreach Teams.
- Benefit linkage/SSI — SSI Application help for those at risk and/or unhoused. Ryan White Case Management and Services offered for patients/clients living with HIV in Yolo County.
- Case management at Mercy Housing — On-site services offered for residents of Mercy Housing in West Sac.
- CalWORKS Program — Therapy for recipients of CalWORKS benefits.
Youth/Family Services and Programs
- Individual counseling, skill development and case management
- Substance use assessment and counseling
- Child Psychiatry
- Specialty mental health (EPSDT)
- Elevate Queer Yolo — Peer Advocacy and case management support for two Spirit/LGBTQ+ ages 12–26.
- K–12 school counseling — Referrals made by schools within the Davis Joint Unified School District and Cesar Chavez High School.
- Woodland Community College school counseling — Counseling and primary care services for enrolled students.
- Specialty mental health (EPSDT) — Services for youth up to 20 years old (Medi-Cal recipients).
- Parent Child Interaction Therapy — For children 7 and under (Medi-Cal recipients).
- Wraparound services — Referrals made by Yolo County Juvenile Probation and Child Welfare.
Prenatal
For pregnant and postpartum individuals, we offer several Behavioral Health programs, including:
- Journey HOME Program, which is a Perinatal substance use treatment program with childcare
- Road to Resilience Home Visiting Program — Free support services to homes with infants under 12 months old.
- In-home Therapy for Caregivers Program — Support throughout pregnancy, up to five years postpartum.
Culturally-responsive services for Spanish-speaking individuals
Our Promotores program offers Education, activities, and resources promoted throughout Yolo County for Spanish-speaking indivuduals.
